Send the request on page 3, then log the case number on page 5.
Who to ask
Your CSS, or wing FSS if you have no CSS. Your state JFHQ for discharge orders and the NGB Form 22. myFSS and the Total Force Service Center (1-800-525-0102) for ARPC records. A free accredited Veterans Service Officer for VA records. Dual-status technicians: your state HRO for the civilian side. This kit organizes your request. It does not change your record or decide eligibility.
Help keep this accurate
Rules, systems, and offices change, and the people doing the work see it first. If something in this kit is outdated, works differently in your state, or is missing, send me a note. Include the tool name, what to change, and a source if you have one, such as a myFSS article, a regulation, or the office that told you. Please never include your SSN, DoD ID, or medical details.
Version 9, checked October 12, 2026.
The fine print
This is an unofficial planning and organization aid. It is not legal, medical, financial, benefits, retirement, or VA claims advice, and it is not an official government form. Not affiliated with or endorsed by the USAF, ANG, or DoD. Contacts and system names were checked in October 2026 and change over time. Verify with your CSS, wing FSS, state JFHQ, ARPC, or the appropriate official source.
